QUOTATION/ORDER FORM
The Dostek Quotation/Order Form is an Excel spreadsheet that incorporates an interactive price list and self-service quotation and order form (Requires Microsoft Excel 2000 and later). To use the form, click the following link and follow the on-screen instructions. You may use the form to view prices, prepare and print multiple 'quotations' for price comparison, and print an order form to submit to Dostek by mail or fax.
PLACE AN ORDER
Order by Phone. Phone orders and inquiries are welcomed. Please call 1-888-355-5735 (toll free, USA and Canada) or 519-742-5721 between 8am and 5pm (M-F, eastern time zone).
Order by Fax. Please send fax orders to 519-741-8669. The Order Form (Excel spreadsheet) may be used for credit card orders. Space is provided for your credit card information and cardholder signature.
PAYMENT METHODS
Dostek is pleased to accept the following methods of payment for your order:
Credit/Debit Card. Visa and Mastercard payments are accepted for orders shipped to USA, Canada, and selected international destinations. Please order by telephone or fax (contact Dostek). For your security, please do not send credit card information by email.
Credit card payments (Visa, Mastercard, American Express and Discover) are also accepted using the PayPal secure online payment system (see below). A PayPal account is not required.
PayPal. PayPal online payments (www.paypal.com) are accepted for orders shipped to USA, Canada and selected international destinations. A PayPal account is not required. Visa, Mastercard, American Express, Discover and PayPal balance payments are accepted. Please order by telephone, fax or email (contact Dostek), and include your email address. You will receive an electronic invoice by email, which you may pay on the PayPal secure website.
Corporate purchase order. Subject to prior credit approval by Dostek. For approval, please contact Dostek.
Bank Funds Transfer. Inter-bank wire payments are the preferred method of payment for orders delivered outside USA and Canada. Please contact Dostek for details.
